Sheehan (SDS) - Sheehan Disability Scale
The Sheehan Disability Scale is a brief, 5-item self-report inventory that assesses functional impairment in work/school, social life, and family life.
Once the patient has submitted it, you my preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your MobileNote.

- Work/School (0-10)
- Social Life (0-10)
- Family Life/Home Responsibilities (0-10)
Total: there is no recommended cutoff score, but this score can be used to monitor change over time with treatment
Individual scores of 5 or more on items 1-3 are associated with significant functional impairment
