C-SSRS - Columbia-Suicide Severity Rating Scale - Screener, Self

Description
The Columbia Protocol, also known as the Columbia-Suicide Severity Rating Scale (C-SSRS), supports suicide risk assessment through a series of simple, plain-language questions that anyone can ask. The answers help users identify whether someone is at risk for suicide, assess the severity and immediacy of that risk, and gauge the level of support that the person needs.
The C-SSRS - Screener, Self-Report, Recent is a shorter version of the C-SSRS that patients can complete for professional review of suicidal ideation and behavior over recent months.

Special Instructions
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.
