SEEK-PSQ - SEEK Parent Screening Questionnaire
The SEEK Parent Questionnaire is a 20-item parent self-report instrument that is part of the broader SEEK program, a comprehensive and practical office-based approach to the identification and assessment of risk factors for child maltreatment, integrated into the delivery of well child health care.
Once the patient has submitted it, you may preview it on the uninitialed documents tab in the EMR. However, do not initial it or it won't later be pulled into your Mobile Note.
